   Sammanfattning : Background: People with a chronic illness such as asthma / COPD (chronic obstructive pulmonary disease), diabetes and congestive heart failure/hypertension are now a days returning to home from hospital a lot earlier than before and therefore need more support, advice, and help from the primary care nurse to be able to manage the disease Aim: The aim of the study was to describe the nurse´s strategies in the meeting with the patient in order to create the best preconditions for a good self-care.
