Att sätta plåster på brutna ben - Om kuratorer, känsloarbete och etisk stress i barn- och ungdomspsykiatrin

Sammanfattning: During the last decades the Swedish Child and Adolescent Psychiatric Services (CAP) has undergone major organizational changes. According to social research the changes are mainly due to new public management reforms. The aim of this study was to examine how social workers, i.e. psychiatry counsellors, experienced and coped with their working conditions within CAP. The method chosen for this study was qualitative interviews with four professional counsellors from three different child psychiatric units in southern Sweden. The interviews were analyzed with the help of Hochschild’s theory about emotional labour and philosophical theories about moral distress. Thematic analysis was chosen as the processing method of data. Results showed that emotional management was a central for the counsellors establishment and maintenance of successful and meaningful relationships with the patients. However, the organizational framework caused difficulties for the emotional work. In order to fulfil their professional standards and to manage moral distress the counsellors used different strategies. The most dominant strategy was identified as “compensatory” to occurring organizational changes. Within limits of inadequate resources, the counsellors used their own emotions, time and ambitions to perform their work in accordance with their identity and judgments as professionals. The study highlights emotional labour as a significant source of moral distress that occurs when the internal environment of the counsellors is incompatible with the needs and prevailing views of the external work environment.
