Patientkontrollerad analgesi (PCA): ett personcentrerat förhållningssätt
Sammanfattning: Background: Analgesia is something nurse anesthetists tackle daily. Different approaches to address the problem exist: the concept of an Acute Pain Service team, multimodal analgesia and patient-controlled analgesia (PCA). Patients report better results with PCA, but problems exist –such as increased risk of respiratory depression and increased mortality. This occurred due to lack of observation. There is a limited amount of research material concerning patients’ experiences of being treated with PCA. Aim: The aim of this study was to describe patients’ experience of postoperative pain treatment with PCA. Method: A quantitative questionnaire covering the following topics was used: ease of use and satisfaction, participation, nurse support and monitoring. 19 patients participated –4 patients in the intervention group and 15 patients in the control group. The patients in the intervention group were given wireless monitoring PCA pumps, while the patients in the control group were given PCA pumps without wireless monitoring capability. A descriptive analysis was done. Results: The patients reported that the PCA pump was easy to use and that with its usage, pain could be better managed. Most patients felt that their participation in their care could have affected their pain treatment and that the physical presence of the nurse had increased this feeling. However, patients thought that having a nurse administer the drugs would not have decreased the pain levels. A great part of the patients estimated their average level of pain to >3, but all were satisfied with the level of pain they experienced. Conclusion: Most patients thought that pain treatment via PCA-pump was a good alternative. The great majority of them thought that wireless monitoring would mean better pain treatment. PCA-pump treatment gives more consideration to the patients’ autonomy, while they play a more active role in their care. In that way, person-centered care is promoted.
