Sammanfattning: Background: The number of young people today living with overweight and obesity and requiring long-term treatment has increased. This requires the care to treat the youth with respect and knowledge. As more and more young people bring their obesity into adulthood, there needs to be a structured transition program in the transition from pediatric to adult care. A transition program should focus on maximizing youth potential and strengthening self-management ability. The optimal goal of a structured transition program is to offer healthcare that is coordinated, uninterrupted, comprehensive and developmentally adapted. There is knowledge of how the transition process for young people with obesity and obesity is implemented in Swedish health care is limited. Aim: The purpose of the study is to describe pediatric nurses' experience of the transition process when youth with overweight and obesity are transitioned from pediatric to adult care. Method: Quantitative survey with pediatric nurses with experience in caring for overweight and obese youth with a descriptive analysis. The data collection was conducted with web-based surveys consisting of 11 multiple-choice questions and one open question. Results: The majority of child nurses in the study state that their unit lack a formal transition program and a transition coordinator. The main obstacles to an organized transition process were lack of resources and economics, limited by time and the absence of a transition coordinator. According to the results, youth should receive support to develop their independence and self-care skills. The majority of the respondents still believe that the youth should be transferred to adult care after reaching a certain age, which is considered to be between 16 and 18 years of age. Conclusions: There are several obstacles to an organized transition process, which is related to lack of time and resources. The transition should take place at the age of 18 and there is difficulty in knowing who will take care of the youth.
