Sammanfattning: Background:Intensive care nurses work in a high-tech environment and are responsible for caring for the critically ill patients with failure of one or more organs, minimizing risks and preventing complications. Sweden lacks specialist nurses, and there is shortage of space that requires shortcomings in the work environment, stress, poorer patient safety and the risk of injuries. The intensive care nurse must take a person-centered approach and preserve the patient's autonomy and integrity. Obstacles to performing nursing according to ethical norms and values can lead to ethical stress. There is an ongoing pandemic (SARS-CoV-2) where patients become seriously ill and need intensive care. Specialist nurses with various cutting-edge skills have been called in to work in a temporary field hospital, IVA Kronan.Aim: To describe specialist nurses' experiences of caring for the critically ill patients in a field hospital, during the pandemic (SARS-CoV-2).Method: A qualitative method with an inductive approach. An appropriate selection consisting of ten specialist nurses from a hospital in Sweden. Interviews were analyzed with a thematic, inductive analysis.Results: The result is presented in four main categories and ten subcategories. The four main categories that emerged were "A demanding work environment", "Inadequate barrier care", "Reduced level of care" and Teamwork in crisis ". The study shows difficult circumstances in which the specialist nurses worked. They described the level of care in the tent was reduced. The specialist nurses described that evidence-based safe care was difficult to fulfill and that the care was prioritized away due to lack of time. Specialist nurses experienced ethical stress and there was a desire for outreach activities for supportive conversations and debriefing. Despite all the prevailing shortcomings, the prevailing pandemic, the brief introduction, and the fact that the staff were called in from different places and with different skills, the staff managed to mobilize force.Conclusion: The study shows the importance of equipping healthcare in the face of a pandemic. If, in Sweden, there are no existing premises to use and the field hospital IVA Krona is used again, the study shows shortcomings that need to be remedied. At the same time, it shows the importance of preparing and training a team and offering personal supportive conversations and debriefing.
