Sjuksköterskans förhållningssätt och strategier vid vård och behandling av patienter i den tidiga palliativa fasen av sin cancersjukdom

Sammanfattning: Background: Patients with an incurable advanced cancer can today live longer, because of increasing effectiveness of chemotherapy or other treatments. However, these patients can quickly be degraded when the treatments are given longer within a palliative phase. Early studies show that communication about prognosis begins late, when patients already has reached a breaking point to the end-of-life. We believe that nurses´ planning and communication are essential to begin earlier for those patients.Aim: The aim was to describe the approaches and strategies that the nurse use in the care and treatment of patients with an advanced cancer in an early palliative phase.Method: A qualitative study using structured interviews with nurses working in oncology day-care/reception and ward. Qualitative content analysis was used.Results: The results were presented as two categories, the nurse´s approaches and the nurse´s strategies. In their approach to the patients, the nurses believed that presence, openness, trust and honesty were important. The patients could be involved in treatment regulations and planning. The nurses´ strategies covers actions to plan for future action, information about treatment and symtoms and to make assessments.Conclusions: The nurses´ approaches that is characterized by presence, openness, trust and honesty may represent a therapeutic relationship in a person-centred care. They were able to describe different approaches and strategies to make the patients involved in decision-making. Nurses felt a great responsibility to discuss the pros and cons of treatment during a change in the patients´ general condition. Most of the nurses´ thought it was important to plan for action early in the patients´ disease, but often this was difficult to implement because of delays in the communication.
