Sammanfattning: Collaboration on paper - a study of how primary health care and social services collaborate by using coordinated individual plans The aim of this study was to analyze how primary health care and social services collaborate by using coordinated individual plans (SIP). Since 2010 health care and social services in Sweden are assigned responsibility to establish coordinated individuals plans for clients and patients in need of coordination of interventions. This study was based on eight semi-structured interviews with coworkers in primary health care and social services. New institutional theory has been used as theoretical framework for the analysis. The result of this study shows that the cowor-kers have a positive attitude towards coordinated individual plans. Nevertheless, it seems like the collaboration with these plans is limited between primary health care and social services. Several obstacles have been identified based on following aspects/circumstances in this work: resources, knowledge, the imperative element of SIP and organizational perspectives. In con-clusion, the study shows that the intentions of the coordinated individual plans are positive, even though there is a lot to be done before primary health care and social services are ready to use these the plans according to the legislator's intentions.
