En stark och betydelsefull händelse Barnmorskors upplevelse av att arbeta med planerad hemförlossning

Sammanfattning: I Sverige är det en kvinna på tusen som väljer planerad hemförlossning. Med planerad hemförlossning menas en förlossning som planeras ske i det födande parets hem med stöd av en legitimerad barnmorska eller läkare. Då hemförlossning idag inte ingår i det svenska hälsovårdssystemet får kvinnan själv se till att ordna ett avtal med barnmorska eller läkare och även bekosta vården själv. Det finns forskning om hemförlossning i relation till medicinsk säkerhet och kvinnors upplevelser. Vidare finns ett fåtal studier om barnmorskor som bistår vid hemförlossning dock inga från Sverige. Syftet med denna studie var därför att beskriva barnmorskors upplevelse av att arbeta med planerad hemförlossning i Sverige. En fokusgruppsintervju utfördes med fyra barnmorskor som arbetar med planerad hemförlossning. Intervjumaterialet analyserades med kvalitativ innehållsanalys. Resultatet presenteras i fyra huvudkategorier. Förlossningen som en betydelsefull händelse där barnmorskorna beskriver förlossningen som en stark upplevelse för såväl det födande paret som barnmorskan. Ostördhet som innefattar vikten av ostördhet för såväl den födande kvinnan som barnmorskan. Att få utöva hantverket som innebär möjligheten att använda sig av och utveckla det traditionella och praktiska yrkeskunnandet. Att vara utanför sjukvårdssystemet som innebär konsekvenser för barnmorskornas arbetsvillkor.In Sweden one in 1000 women chooses planned homebirth. The definition of a planned homebirth is a birth that is planned to take place at home with support from a registered midwife or physician. Planned homebirths are not included in the Swedish healthcare system. The woman has to make a deal with a midwife or a physician, and pay for the care herself. There is research about homebirth related to medical safety and women´s experiences, however only a few studies about midwives experiences of working with homebirth, and none of them from Sweden. The aim of this study was to describe midwives experiences working with planned homebirth. One focus group interview was conducted with four midwives with work experiences of planned home births. The interview were analysed with content analysis. The result is presented in four categories. The delivery as a meaningful moment where the midwives describes the delivery as a powerful experience for both the becoming parents and themselves. Privacy including the importance of privacy for both the woman giving birth and the midwife. To practice the craft that means the ability to use and develop the traditional and practical professional skills. Not to be part of the healthcare system causing consequenses for the midwives working conditions.
