"Oavsett hur bra vi än samverkar" : En studie om samverkan mellan sjukvård och kommun vid utskrivningar från psykiatrisk slutenvård

Sammanfattning: This study examines collaboration between social services, inpatient- and outpatient psychiatric care regarding to discharge from psychiatric inpatient care. The research questions examined in the study were how professionals working in these organizations perceive the discharge process with regards to limiting factors, facilitating factors and their experiences of how the patient have experienced discharge. The study is based on two focus group interviews with professionals working in psychiatric inpatient- and outpatient care and three individual interviews with professionals working in the social services. The results show that facilitating factors for collaboration are clear communication, when patients already have contact with the social services since before, and when the professionals already have an established professional relationship. Poor communication, unclear responsibilities, and lack of respectfor each individual profession, were the main limiting factors. The legislative requirement for a fast discharge process was a limiting factor due to social care efforts long lead time.
