Sammanfattning: The aim of this study was to ascertain how school counselors experience the transition from child and youth psychiatry to adult psychiatry that their students go through when they turn 18 years old. The study also aimed to discover the meaning behind the transition in relation to the school counselors’ work situation. We chose to limit our sample of respondents to school counselors at the upper secondary level in Skåne, Sweden. With a qualitative approach and semi-structured interviews as the method of the study, we gathered the empirics for our analysis. The data was analysed based on Afaf Meleis’ theory of transition, Andrew Abbott’s concept of jurisdiction, and the concept of professional uncertainty. The results of the study showed that the school counselors mostly had negative experiences of the transition. It often resulted in a time gap, a lack of safe care, and uncertainty in the process for the students, which had a negative effect on their health. The changes in the student's well-being in addition affected the work situation of the school counsellors. The counselors had different strategies to manage the situation but it became clear that it was difficult for them to set limits against students, the school management, the psychiatrists’ and themselves.
